Who May Qualify?
A child may qualify if they are under 21, have active Georgia Medicaid, have a medically fragile condition requiring skilled care, have a physician’s order documenting medical necessity, and can safely receive care at home.
Common qualifying diagnoses or needs may include:
- Tracheostomy care
- Ventilator dependence
- Feeding tubes (G-Tube)
- Seizure disorders
- Cerebral palsy
- Chronic respiratory conditions
- Neurological disorders
- Premature infants with ongoing medical needs
- Other complex medical conditions
